In the initial room with the enemies fighting each other, drop down to the lower level filled with scarlet rot and look for the chest, it will be in the center of the floor of the West most wall. This catacomb is located in the northmost edge of Starscourge Radahn arena after beating him, just south of Lenne's RiseGrace.
