You must have killed Lichdragon Fortissax in Fia's Dream and received the Mending Rune of Death.
can be acquired slightly earlier if you kill D when he appears in LimgraveRegion, close to the Summonwater Village OutskirtsGrace.
Doing this allows you to give his armor to his brother in Siofra AqueductLandmark and after reloading the area, the brother will be hostile to you.
Kill him to get the Inseparable Sword and Twinned Armor back.
This does not break Fias questline.
